Palau at a glance

Palau’s metabolic-health story brings together rapid food transition, a nationally measured burden of diabetes and prediabetes, and an unusually strong opportunity to reconnect public health with taro agriculture, fisheries, women’s knowledge and tourism. Koror concentrates much of the population and retail food environment, while Babeldaob and the outlying states hold land, farms and food traditions essential to long-term resilience.

What the national data show

Palau’s WHO STEPS survey is one of the most useful national datasets in Micronesia because it combines questionnaires with physical and biochemical measurements. A peer-reviewed analysis of 1,915 adults aged 25–64 reported age-standardized diabetes prevalence of 17.7% and prediabetes prevalence of 40.4%. Together, those findings showed that abnormal glucose regulation affected a large share of working-age adults at the time of the survey.

The study also found that diabetes occurred at a younger age among adults with obesity than among adults without obesity. Central adiposity, hypertension and elevated triglycerides were important accompanying signals. These findings support integrated screening: glucose and HbA1c should be considered alongside waist measurement, blood pressure, lipids, kidney function, urine albumin, liver health and serum urate.

The estimates come from survey data collected before publication in 2016. They remain important, but they are not a substitute for a new, nationally representative measured survey. Palau should be able to show whether prevention, treatment and food-policy efforts have changed the trajectory.

Taro is food, land, knowledge and identity

Palauan food heritage is built around taro and giant swamp taro, breadfruit, yam, cassava, sweet potato, banana, coconut, fish, shellfish and other marine foods. Taro patches are not simply crop fields. They embody water management, land stewardship, women’s agricultural expertise, family exchange and ceremonial life.

Palau’s national plant-genetic-resources report identifies root crops—especially taro and giant swamp taro—as major traditional staples. Protecting named cultivars and planting material therefore has several benefits at once: dietary diversity, climate resilience, cultural continuity and reduced dependence on imported refined starches.

From taro patch to contemporary market

FAO has documented Palauan women developing taro flour and learning to use it in pasta, noodles, sauce, cookies, bread and brownies, together with improved packaging. This work matters because convenience determines what survives a food transition. A root crop that requires lengthy preparation competes poorly with instant noodles or packaged bread; locally produced flour, frozen portions and clearly labeled prepared foods can bring taro into modern schedules.

The metabolic value of a taro product depends on the recipe. A bread or cookie containing a small amount of taro flour can still be dominated by refined wheat flour, added sugar and fat. Product development should therefore report ingredient percentages, fiber and added sugar, while gradually increasing the amount of whole taro or taro flour. The goal is both cultural visibility and meaningful nutritional improvement.

Tourism can strengthen—or bypass—the local food economy

Palau’s tourism economy creates demand for meals every day. Hotels, restaurants, tours and events can purchase taro, breadfruit, fruit, vegetables and responsibly harvested seafood from Palauan producers. FAO’s agritourism work emphasizes that visitors can help keep food traditions and agricultural practices economically active.

Procurement is the crucial bridge. Publishing the proportion of institutional and tourism food spending that reaches local growers and fishers would turn “eat local” from a slogan into a measurable development strategy. Training, food-safety support, aggregation, cold storage and reliable contracts can help small producers meet commercial demand.

The imported retail environment

Palau depends on imported food, and imports are essential for choice and food security. The health problem emerges when routine retail shelves, restaurants and convenience outlets make sugary beverages, refined rice, white-flour products, processed meat and packaged snacks easier and cheaper than water and minimally processed meals.

Palau has previously applied an import tariff to carbonated soft drinks. Fiscal policy works best when paired with visible alternatives, school standards, front-of-pack information, public procurement and investment of revenue in safe drinking water, local agriculture and prevention. A tax should be evaluated by changes in purchasing and health—not merely by revenue collected.

Fructose, liver fat and uric acid

Sugar-sweetened beverages deliver added sugar quickly and with little satiety. Rapid fructose phosphorylation in the liver consumes ATP; AMP degradation can increase uric-acid production, while excess substrate can contribute to liver fat, elevated triglycerides and insulin resistance. Variants in urate-transport genes, including SLC2A9, which encodes GLUT9, can modify susceptibility to hyperuricemia and gout.

Uric acid is relevant beyond gout. Crystal deposition causes gout and can contribute to kidney stones and kidney injury, while elevated urate often accompanies metabolic dysfunction and reduced kidney clearance. Individual assessment should connect serum urate with kidney function, urine albumin, blood pressure, glucose and medication review.

Schools and young people

Children encounter the food transition through beverages, snacks, school meals, advertising and family purchasing. WHO’s school-food guidance supports nutrition standards for foods and drinks served or sold at school, direct provision of healthy meals, and changes that make healthier choices easier.

For Palau, a strong school environment would make safe water universally available; exclude routine sugary-drink sales; limit added sugar and highly processed products; and create practical purchasing pathways for local fruit, vegetables, taro and fish. Student acceptance, waste, cost and local procurement should be measured and published.

A practical Palau pathway

  • Repeat national measurement: update STEPS data and report results by age, sex and relevant population groups.
  • Protect taro systems: invest in water management, planting material, women farmers and climate-resilient cultivation.
  • Build convenient local products: develop taro and breadfruit foods with transparent ingredients and less added sugar.
  • Use tourism purchasing power: set measurable local-procurement goals for hotels, restaurants and events.
  • Make water the default: align schools, government facilities and health services around unsweetened beverages.
  • Link screening to care: ensure abnormal glucose, blood pressure, kidney, lipid, urate and liver results receive continuing follow-up.
